When your provider is part of Partnership in Health, you can expect to see:

No changes to your existing Medicare.

You keep your traditional Medicare rights, coverage and benefits, including the freedom to see any Medicare provider. You or your loved one still have the right to visit any doctor, hospital or other provider that accepts Medicare at any time, just like you do now. Your doctor may make recommendations, but it’s always your choice on what doctors or hospitals you utilize. Partnership in Health will consider not only medical conditions, but issues related to health literacy, advanced care planning, informed consent, personal health trajectory, prognosis and overall health goals.

Partnership in Health is part of a new program from CMS called ACO REACH, which aims to provide Medicare beneficiaries with access to enhanced benefits and to increase the availability of high-quality, coordinated care, including for people in underserved populations. Through Partnership in Health, the provider who takes care of you in a CommuniCare nursing facility works in partnership with the nursing facility to coordinate your care.

ACO REACH stands for Accountable Care Organization for Realizing Equity, Access, and Community Health. This model is part of a new CMS innovation program focused on promoting health equity and addressing healthcare disparities for underserved communities. It is a redesigned version of the Global and Professional Direct Contracting Model (GPDC) Model. The model provides healthcare providers with resources that empower them to improve the quality of care they're able to offer traditional Medicare patients.

No. Your benefits will NOT change, and you can visit any doctor, other healthcare professional or hospital. You are still eligible for the same Medicare benefits, and you still have the right to use any doctor, other healthcare professional or hospital that accepts Medicare.
