About the ACO REACH model
The model is part of the CMS Innovation Center and aims to test ways to address health inequities among Traditional Medicare beneficiaries.
Nursing home residents are often individuals who have historically been underserved in a community setting, enduring health inequities driven by social determinants of health such as food scarcity, medication access and adherence, and transportation and access limitations. Partnership in Health sees ways that a resident-focused care model addresses these historic inequities every day, and opportunities to expand and formalize this care through joint accountability and collaboration.
